Phoenix Home Care & Hospice is seeking an Registered Nurse for in-home Private Duty Care and VA departments, covering Jefferson City, MO and surrounding areas. The role involves assessments, reassessments, and routine nursing visits including medication setup and plane of care maintenance.

The position offers per-visit compensation and opportunities to serve wounded warriors and veterans in the comfort of their own homes, with services funded privately, by long-term care insurance, or VA.

Qualifications

• Active MO RN or LPN license required.
• In-home care experience preferred.
• Strong organizational and self-management skills.
• Excellent customer service skills.
• Valid driver's license required.

Responsibilities

• Provide basic patient care including vitals and medication management per physician orders.
• Open new cases and obtain related consents, documentation and deposits per company policy.
• Observe, record and report reactions to treatment and any changes in the client’s condition to appropriate personnel/physician.
• May supervise In-Home Aides for Advanced Personal Care (APC) services.
• Provide instruction/education on medications when needed.
• Follow state-approved care plans.
• Attend quarterly nurse meetings.
• Demonstrate good time management and be detail-oriented.

• Part Time and PRN Availability- HCY, Private Care Nurse & Veteran Affairs Nurse
• Jefferson City, MO and surrounding areas
• Pay Range: $30-$40 per visit

We are currently seeking a Registered Nurse for our Private Duty Care and VA departments to cover In-Home visits and to work with our Healthy Children & Youth program performing assessments and reassessments. This position will be responsible for aide oversight visits and routine nursing visits; including medication set ups and maintaining the plane of care. In this RN role, you will have the opportunity to make a difference in the lives of our clients by providing high-quality care and support in the comfort of their own home. These services are either privately paid, funded by a Long-Term Care Insurance policy, or the VA.

Responsibilities:

• Provide basic patient care: Taking Vitals, Weekly medication Management per Physicians orders, Monthly Diabetic Nails, Monthly Skin Monitoring and Bi Yearly General Health Assessments
• Opens new cases & obtains all related consents, documentation & deposits in accordance with company policy
• Observes, records & reports reaction to treatment & any changes in client's condition to appropriate personnel and/or physician

Requirements:

• Active Registered Nurse or Licensed Practical Nurse license in the state of MO
