Job summary

Phoenix Home Care is seeking an In-Home Registered Nurse or Licensed Practical Nurse to join our Springfield, MO team. You will deliver high-quality in-home nursing care to adults and seniors, improving comfort and health outcomes in the client’s home environment.

Qualifications

• Active Registered Nurse or Licensed Practical Nurse license in MO.
• Home care experience preferred but not required.
• Strong organizational and self-management skills.
• Strong and compassionate customer service skills.
• Valid driver's license.

Responsibilities

• Perform admission and reassessment visits for in-home clients.
• Conduct nurse visits including vitals, medication management, and routine diabetic care.
• May supervise caregiver visits and coordinate care.
• Educate clients and families on medications and health management.
• Demonstrate time management, attention to detail and initiative.

Responsibilities:

• Complete admission assessments and reassessments for In-Home caregiver population.
• Provide nurse visits for adult and senior population: Taking vitals, medication management, monthly diabetic nails, and labs.
• May provide supervisory visits for caregiver services
• Provide instruction/ education on medication
• Good time management, detail oriented and self-starter

Requirements:

• Active Registered Nurse or Licensed Practical Nurse license in the state of MO
• Home Care experience preferred but not required
• Strong organizational and self-management skills
• Strong and compassionate customer service skills
• Valid driver's license
