Job summary

Phoenix Home Care & Hospice seeks a Registered Nurse to cover Private Duty Care and VA departments for in-home visits in Topeka, KS and surrounding areas. The RN will perform assessments, medication setups, and plan of care maintenance, with pay per visit ($30–$40).

The role includes oversight visits for aides and interaction with a home health team. Applicants should hold an active KS RN license, have in-home care experience preferred, and possess strong organizational and customer service

Qualifications

• Active Registered Nurse license in KS required.
• In-Home nursing experience preferred.
• Strong customer service and communication skills.
• Good time management and organization.

Responsibilities

• Provide basic patient care during in-home visits (vital signs, medication management per physician orders).
• Perform assessments and reassessments; participate in care planning and documentation.
• Supervise In Home Aides for APC services if needed.
• Educate patients and caregivers on medications and care plans.
• Follow state-approved care plans and attend nurse meetings.
• Maintain accurate records and report changes in patient condition to the physician.

• Pay Range: $30-$40 per visit

We are currently seeking a Registered Nurse for our Private Duty Care and VA departments to cover In-Home visits and to work with our Healthy Children & Youth program performing assessments and reassessments. This position will be responsible for aide oversight visits and routine nursing visits; including medication set ups and maintaining the plane of care. In this RN role, you will have the opportunity to make a difference in the lives of our clients by providing high-quality care and support in the comfort of their own home. These services are either privately paid, funded by a Long-Term Care Insurance policy, or the VA.

Responsibilities:

• Provide basic patient care: Taking Vitals, Weekly medication Management per Physicians orders, Monthly Diabetic Nails, Monthly Skin Monitoring and Bi Yearly General Health Assessments
• Opens new cases & obtains all related consents, documentation & deposits in accordance with company policy
• Observes, records & reports reaction to treatment & any changes in client's condition to appropriate personnel and/or physician
• May supervise In Home Aides for Advance Personal Care (APC) Services
• Provide instruction/ Education on Medication when needed.
• Follow state approved care plan.
• Attend quarterly Nurse Meetings
• Good time management, detail oriented and self-starter

Requirements:

• Active Registered Nurse license in the state of KS
• In Home experience preferred
• Strong organizational and self-management skills
• Strong and compassionate customer service skills
• Valid driver’s license
