Job summary

Phoenix Home Care & Hospice in Topeka, KS is seeking a Registered Nurse for Private Duty Care and VA departments to cover in-home visits and perform assessments and reassessments. This RN role will involve medication setup and maintaining the plan of care in clients' homes.

You will have the chance to impact veterans and other clients by delivering compassionate, high-quality home health care through our Healthy Children & Youth program and private-pay or funded services.

Qualifications

• Active Registered Nurse license in KS.
• In Home experience preferred.
• Strong organizational and self-management skills.
• Strong and compassionate customer service skills.
• Valid driver’s license.

Responsibilities

• Provide basic patient care: taking vitals, weekly medication management per physicians orders, monthly diabetic nails, monthly skin monitoring and bi yearly general health assessments.
• Opens new cases & obtains all related consents, documentation & deposits in accordance with company policy.
• Observes, records & reports reaction to treatment & any changes in client's condition to appropriate personnel and/or physician.
• May supervise In Home Aides for Advance Personal Care (APC) Services.
• Provide instruction/ Education on Medication when needed.
• Follow state approved care plan.
• Attend quarterly Nurse Meetings.
• Good time management, detail oriented and self-starter.

• Full Time Availability- HCY, Private Care Nurse & Veteran Affairs Nurse
• Topeka, KS and surrounding areas
• Pay Range: $30-$40 per visit

We are currently seeking a Registered Nurse for our Private Duty Care and VA departments to cover In-Home visits and to work with our Healthy Children & Youth program performing assessments and reassessments. This position will be responsible for aide oversight visits and routine nursing visits; including medication set ups and maintaining the plane of care. These services are either privately paid, funded by a Long-Term Care Insurance policy, or the VA.
