Job summary

Phoenix Home Care & Hospice is seeking an RN for private care nurse duties in Lebanon, MO and surrounding areas. This part-time role features weekday visits with no on-call and compensation up to $45 per visit.

The nurse will provide basic patient care, manage medications per physician orders, supervise in-home aides, educate on medications, and follow a state-approved care plan, with quarterly nurse meetings and a focus on compassionate, autonomous service.

Qualifications

• Active RN license in MO.
• In-home experience preferred.
• Strong organizational, time-management and self-management skills.
• Reliable transportation and valid driver’s license.
• Patient-focused, compassionate customer service.

Responsibilities

• Take vitals and manage weekly medication under physician orders.
• Monthly diabetic nails and monthly skin monitoring.
• Bi-yearly general health assessments.
• May supervise In Home Aides for APC services.
• Provide instruction/education on medication.
• Follow state-approved care plan.
• Attend quarterly nurse meetings.

Responsibilities:

• Provide basic patient care: Taking Vitals, Weekly medication Management per Physicians orders, Monthly Diabetic Nails, Monthly Skin Monitoring and Bi Yearly General Health Assessments
• May supervise In Home Aides for Advance Personal Care (APC) Services
• Provide instruction/ Education on Medication when needed.
• Follow state approved care plan.
• Attend quarterly Nurse Meetings
• Good time management, detail oriented and self-starter

Requirements:

• Active RN license in the state of MO
• In Home experience preferred
• Strong organizational, time management, and self-management skills
• Trustworthy, dependable, and able to work autonomously
• Strong and compassionate customer service skills
• Valid driver's license, Active Auto Insurance, and Reliable Transportation
