Job summary

Phoenix Home Care and Hospice is hiring a Registered Nurse in Springfield, MO, to provide private duty care and support to clients in their homes. This role involves comprehensive patient assessments, medication management, and supervising aides. Candidates must have an active RN license in Missouri and preferably have in-home care experience.

Qualifications

Responsibilities

• Provide basic patient care including taking vitals and medication management.
• Open new cases and obtain all necessary documentation.
• Observe, record, and report changes in the client's condition.
• Supervise In Home Aides for Advance Personal Care Services.
• Provide education on medication when necessary.
• Follow the state-approved care plan.
• Attend quarterly Nurse Meetings.

Full Time and PRN Availability- HCY, Private Care Nurse & Veteran Affairs Nurse
Springfield, MO and surrounding areas
Pay Range: $30-$40 per visit

We are currently seeking a Registered Nurse for our Private Duty Care and VA departments to cover In-Home visits and to work with our Healthy Children & Youth program performing assessments and reassessments. This position will be responsible for aide oversight visits and routine nursing visits; including medication set ups and maintaining the plane of care. In this RN role, you will have the opportunity to make a difference in the lives of our clients by providing high-quality care and support in the comfort of their own home. These services are either privately paid, funded by a Long-Term Care Insurance policy, or the VA.

Responsibilities

• Provide basic patient care: Taking Vitals, Weekly medication Management per Physicians orders, Monthly Diabetic Nails, Monthly Skin Monitoring and Bi Yearly General Health Assessments
• Opens new cases & obtains all related consents, documentation & deposits in accordance with company policy
• Observes, records & reports reaction to treatment & any changes in client's condition to appropriate personnel and/or physician
• May supervise In Home Aides for Advance Personal Care (APC) Services
• Provide instruction/ Education on Medication when needed
• Follow state approved care plan
• Attend quarterly Nurse Meetings
• Good time management, detail oriented and self-starter

Requirements
