Job summary

Human Touch Home Health is seeking a compassionate LPN to join our team serving Richmond and surrounding communities. As a Home Health LPN, you'll provide skilled nursing care in patients' homes, manage chronic conditions, and support recovery with personalized care.

You'll have flexibility with PRN scheduling, the autonomy to build meaningful patient relationships, and opportunities for professional growth within a trusted, 27-year-old company that values clinicians.

Qualifications

• Active Virginia LPN license required.
• Valid driver’s license and reliable transportation.
• Strong clinical, communication, and documentation skills.
• Home health experience preferred but not required.

Responsibilities

• Provide skilled nursing care under the direction of a Registered Nurse and physician
• Administer medications and perform treatments as ordered
• Monitor patient conditions and report changes to the supervising RN
• Assist with disease management and patient education
• Complete wound care and other skilled nursing procedures within your scope of practice
• Collaborate with physicians and the interdisciplinary care team
• Complete timely and accurate clinical documentation
