Qualifications

• Graduate of an accredited nursing program (required).
• Current and active RN license in New Jersey (required).
• Previous long-term care or assisted living experience (preferred).
• Strong interpersonal and communication skills.

Responsibilities

• Coordinate resident admissions, room changes, and transitions in accordance with community procedures.
• Monitor resident health and wellness through routine observations and ongoing evaluations.
• Deliver professional nursing services that align with individualized care goals and established standards.
• Maintain accurate and up-to-date electronic health records, including care provided and medication administration.
• Participate in resident assessments, care conferences, and the development of personalized support plans.
• Work closely with residents, families, and interdisciplinary team members to encourage a positive resident experience and overall well-being.
