Park Manor Ltd in Park Falls, WI, a 100% employee-owned Rehabilitation & Nursing Care Facility, seeks qualified RNs to provide nursing services to residents per scope of practice and facility policies.

You will monitor residents for acute changes, perform assessments, administer medications, document care in EMR, and maintain clear communication with families, physicians and the care team to ensure continuity and high-quality care.

Qualificações

• Valid & current WI RN license.
• Valid & current CPR certification.

Responsabilidades

• Provide nursing services to residents according to scope of practice and facility policies.
• Monitor residents for acute changes and conduct assessments; notify provider as needed.
• Maintain documentation of nursing care in nurses' notes, flow sheets and EMR.
• Apply effective communication with residents, families, physicians and healthcare team; ensure continuity between shifts.
• Administer and document medications according to practitioner orders; report adverse effects or errors.
• Participate in team care conferences to develop resident goals and nursing outcomes.

Job Summary

• Provide nursing services to residents in accordance with scope of practice, facility policies and professional standards of care
• Monitor residents for developments of acute changes of condition including, but not limited to, confusion, fever difficulty breathing, increased pain, difficulty with urination, loss of appetite, constipation, diarrhea, vision or hearing loss, general malaise or decreased range of motion (ROM); conduct assessments and notify the provider as needed
• Maintain documentation of all nursing care and services provided to the residents; use nurses' notes, flow sheets and electronic medical records according to facility protocol
• Applies effective communication skills in establishing relationships with residents, families, physicians, and other members of the healthcare team. Assures communication and continuity between administration, nurses, CNAs, and ancillary departments, within and between shifts
• Administer and document medications according to practitioner orders and report adverse consequences, side effects or any medication errors
• Participate in team care conferences in developing resident goals priorities, and nursing outcome criteria aimed at meeting the total needs of the resident
