Job summary

Park Manor Ltd in Park Falls, WI, is hiring a Licensed Practical Nurse (LPN) to provide nursing care in a Rehabilitation & Nursing Care Facility. The 12-hour master schedule supports a balanced work-life, with full-time and part-time day/night options.

Responsibilities include administering medications, monitoring residents for changes in condition, documenting care, and communicating across the care team to ensure continuity and quality care.

Qualifications

• Must hold a valid and current WI LPN license.
• Must hold a valid and current CPR certification.

Responsibilities

• Provide nursing services to residents in accordance with scope of practice and facility policies.
• Monitor residents for acute changes and notify provider as needed.
• Administer and document medications per orders and report adverse effects.
• Maintain nursing documentation using notes, flow sheets, and EMR per protocol.
• Foster effective communication with residents, families, physicians, and healthcare team.

LICENSED PRACTICAL NURSE (LPN)

Requirements

Job Summary

• Provide nursing services to residents in accordance with scope of practice, facility policies and professional standards of care
• Monitor residents for developments of acute changes of condition including, but not limited to, confusion, fever, difficulty breathing, increased pain, difficulty with urination, loss of appetite, constipation, diarrhea, vision or hearing loss, general malaise or decreased range of motion (ROM); conduct assessments and notify the provider as needed
• Administer and document medications within the scope of practice and according to practitioner orders. Report adverse consequences, side effects or medication orders
• Maintain documentation of all nursing care and services provided to the residents; use nurses’ notes, flow sheets and electronic medical records according to facility protocol
• Apply effective communication skills in establishing relationships with residents, families, physicians, and other members of the healthcare team. Assures communication and continuity between administration, nurses, CNAs, and ancillary departments, within and between shifts
