Job summary

Human Touch Home Health is seeking a compassionate Registered Nurse (RN) to join our team delivering in-home care across Blacksburg, Christiansburg, Radford, and nearby New River Valley communities.

This PRN role offers flexible scheduling, autonomy in patient care, and opportunities to build a caseload while ensuring high-quality nursing services, wound care, med management, and patient education in patients’ homes.

Qualifications

• Active Virginia RN license is required.
• Valid Driver’s License and reliable transportation.
• Strong assessment and critical thinking skills.
• Excellent communication and documentation abilities.
• Home health experience is a plus but not required.

Responsibilities

• Perform comprehensive patient assessments.
• Develop and implement individualized plans of care.
• Provide skilled nursing, medication management, wound care, and patient education.
• Monitor patient progress and communicate changes to physicians and care team.
• Coordinate care with therapists, social workers, and home health aides.
• Complete timely, accurate clinical documentation.
• Educate patients and caregivers to promote health and independence.

• OASIS experience is a plus
