Job summary

Human Touch Home Health is seeking compassionate Registered Nurses to provide PRN home health visits in Blacksburg and surrounding communities. You will focus on one patient at a time, delivering skilled nursing care in patients’ homes while maintaining high clinical standards.

Ideal candidates hold an active Virginia RN license, have strong assessment and communication skills, and can work independently. WellSky experience and OASIS familiarity are a plus.

Qualifications

• Active Virginia RN license required.
• Graduate of an accredited nursing program.
• Valid driver's license and reliable transportation.
• Strong assessment and clinical decision-making skills.
• Excellent communication and organizational skills.
• Ability to work independently in patients' homes.
• Home Health and OASIS experience preferred, but not required.
• WellSky experience a plus.

Responsibilities

• Complete skilled nursing visits in patients' homes.
• Perform comprehensive nursing assessments.
• Complete Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned.
• Provide wound care and medication management.
• Monitor patients for changes in condition.
• Provide disease-management education.
• Educate patients, families, and caregivers.
• Communicate changes/concerns to physicians and care team.
• Coordinate with therapists and interdisciplinary team.
• Identify concerns early to reduce hospitalizations.
• Complete accurate and timely documentation using WellSky.

What You'll Do

As a Home Health RN, you will:

• Complete skilled nursing visits in patients' homes
