Job summary

Human Touch Home Health in Blacksburg is seeking compassionate Licensed Practical Nurses for PRN home health opportunities. Join a team that values one-on-one patient care and flexible scheduling, with visits in Blacksburg and surrounding communities.

As an LPN, you will perform skilled nursing visits under an RN's direction, manage medications, wound care, and education, while building meaningful relationships with patients in their homes.

Qualifications

• Active Virginia LPN license required.
• Graduate of an accredited Practical Nursing program.
• Reliable transportation and valid driver’s license.
• Excellent clinical and communication abilities.
• Home health experience preferred but not required.

Responsibilities

• Complete skilled nursing visits in patients' homes
• Monitor and document patients' conditions
• Provide medication management and education
• Perform wound care and other ordered treatments
• Monitor vital signs and report changes
• Reinforce disease-management education
• Educate patients, families, and caregivers
• Communicate changes to supervising RN and care team
• Coordinate with interdisciplinary team
• Follow plans of care and physician orders
• Complete accurate documentation using WellSky

What You'll Do

As a Home Health LPN, you will provide skilled nursing care under the direction of an RN and physician-established plan of care, including:

• Complete skilled nursing visits in patients' homes
• Monitor and document patients' conditions
• Provide medication management and education
• Perform wound care and other ordered treatmentsoops
• Monitor vital signs and report significant changes
• Reinforce disease-management education
• Educate patients, families, and caregivers
• Communicate changes in condition to the supervising RN and care team
• Coordinate with other members of the interdisciplinary team
• Follow established plans of care and physician orders
• Complete accurate and timely documentation using WellSky

What We're Looking For

• Active Virginia Licensed Practical Nurse (LPN) License Required
• Graduate of an accredited Practical Nursing program
• Valid Driver's License and reliable transportation
• Strong clinical and communication skills
• Dependable, organized, and comfortable working independently in patients' homes
• Ability to follow established plans of care and communicate effectively with the supervising RN
• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus
