Job summary

Miracle City Hospice in Broadview Heights, OH is seeking an experienced RN Case Manager to lead patient-focused hospice care. You will act as the primary nurse and advocate, ensuring compassionate, coordinated care across the hospice journey.

The role offers autonomy within an interdisciplinary team, opportunities for growth, and a commitment to comfort, dignity, and quality of life for patients and families.

Qualifications

• Graduate of an accredited Registered Nursing program.
• Active RN license in the state of practice.
• Current CPR and First Aid certification.
• Knowledge of quality assessment and quality assurance practices.

Responsibilities

• Manage and coordinate a caseload of hospice patients and families.
• Perform patient assessments, admissions, care planning, and ongoing evaluations.
• Develop individualized care plans addressing physical, emotional, social, and spiritual needs.
• Collaborate with physicians, social workers, aides, chaplains, and other team members.
• Conduct routine patient visits and adjust care plans as needs change.
• Provide education, guidance, and support to patients and their families.
• Perform clinical pronouncements and support during end-of-life transitions.
• Maintain accurate and timely clinical documentation.
• Supervise and support Home Health Aides in line with guidelines.

Schedule: Monday-Friday, 8:00 AM-5:00 PM

Territory: East Side Coverage from Beachwood to Geneva

What You'll Do

• Maintain accurate and timely clinical documentation
• Supervise and support Home Health Aides in accordance with state, federal, and company guidelines

Qualifications

• Graduate of an accredited Registered Nursing program
• Active Registered Nurse (RN) license in the state of practice
• Current CPR and First Aid Certification
• Knowledge of quality assessment and quality assurance practices
• Strong clinical judgment, communication, and patient advocacy skills
• Valid driver's license and proof of insurance
