Job summary

Capital City Hospice in Columbus, Ohio is seeking an experienced RN Case Manager to provide primary nursing care for patients and families under a Monday–Friday schedule (8 a.m.–5 p.m.). You will complete assessments, determine hospice eligibility, lead admissions, and develop individualized plans of care in collaboration with the interdisciplinary team.

Qualifications

• Graduate of an accredited registered nursing program.
• Current RN license in the state of practice.
• Current CPR and First Aid certification.
• Knowledge of quality assessment and assurance practices.
• Strong clinical judgment and ability to make independent decisions.
• Compassionate communication skills and the ability to support patients and families.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Serve as the primary RN for your patient and family caseload.
• Complete assessments and determine hospice eligibility.
• Lead the admission process and develop individualized plans of care.
• Evaluate physical, emotional, social, and spiritual needs and coordinate interventions with the interdisciplinary team.
• Communicate with attending physicians, team members, and partner agencies to ensure seamless care.
• Conduct regular patient visits to reassess needs and update care plans.
• Perform clinical pronouncements of death and provide compassionate support to families.
• Maintain accurate, timely documentation, care plans, and progress notes; supervise and document care provided by Home Health Aides per federal, state, and company guidelines.
