Job summary

Ohio Valley Hospice, affiliated with BrightSpring Health Services, seeks an experienced RN Case Manager in Cincinnati, OH to deliver patient‑centered hospice care. You will assess needs, develop care plans, administer treatments, and support families in challenging times.

Ideal candidates hold an RN license in Ohio with at least one year of nursing experience; hospice exposure is preferred. A valid driver’s license and reliable vehicle are required for community visits.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in Ohio.
• Minimum one year of nursing experience; hospice experience preferred.
• Strong commitment to clinical excellence and accurate documentation.
• Must be a licensed driver with a reliable vehicle.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create and adjust hospice care plans aligned with patient and family goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with interdisciplinary team to coordinate care.
• Maintain accurate and timely documentation in charting systems.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
