Job summary

Queen City Hospice in Ohio is seeking a compassionate Registered Nurse (RN) to join as Hospice RN Case Manager. Manage patient caseload, lead the full nursing process from assessment to evaluation, and deliver holistic end-of-life care in collaboration with an interdisciplinary team.

Qualifications

• Active Registered Nurse license in Ohio (or compact state)
• Graduate of an accredited nursing program
• Current CPR and First Aid certification
• Knowledge of hospice care standards and quality assurance

Responsibilities

• Manage a patient caseload and lead the full nursing process: assessment, admission, care planning, implementation, and evaluation
• Deliver holistic patient care addressing physical, emotional, psychological, social, and spiritual needs
• Collaborate with interdisciplinary care team including physicians, social workers, and chaplains
• Coordinate care with attending physicians and external providers
• Conduct routine patient visits and update plans of care
• Provide end-of-life care and family support
• Maintain accurate documentation in compliance with standards
• Supervise and oversee care delivery by HHAs

Location: Southwest Ohio
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (Only 2 holiday on-call days per year)

Key Responsibilities

As a Hospice RN Case Manager, you will:

• Manage a patient caseload and lead the full nursing process: assessment, admission, care planning, implementation, and evaluation
• Deliver holistic patient care, addressing physical, emotional, psychological, social, and spiritual needs
• Collaborate with the interdisciplinary care team, including physicians, social workers, and chaplains
• Coordinate care with attending physicians and external providers
• Conduct routine patient visits and update individualized plans of care
• Provide end-of-life care, including clinical pronouncement and family support
• Maintain accurate, timely documentation in compliance with state, federal, and company standards
• Supervise and oversee care delivery by Home Health Aides (HHAs)

Qualifications

• Active Registered Nurse (RN) license in Ohio (or compact state if applicable)
• Graduate of an accredited nursing program
• Current CPR and First Aid certification
• Knowledge of quality assurance and hospice care standards
• Valid driver’s license and auto insurance
