Miracle City Hospice in Broadview Heights, OH seeks an RN Case Manager to lead compassionate, patient-centered hospice care. You will coordinate care, assess and plan with an interdisciplinary team, and advocate for patients and families throughout the journey.

The role offers autonomy, strong support from a diverse team, and opportunities to enhance quality of life for patients at end of life.

Qualifications

• Graduate of an accredited Registered Nursing program.
• Active RN license in the state of practice.
• Current CPR and First Aid Certification.
• Knowledge of quality assessment and quality assurance practices.
• Strong clinical judgment, communication, and patient advocacy skills.
• Valid driver's license and proof of insurance.

Responsibilities

• Manage and coordinate a caseload of hospice patients and families.
• Complete patient assessments, admissions, care planning, and ongoing evaluations.
• Develop individualized care plans that address physical, emotional, social, and spiritual needs.
• Collaborate closely with physicians, social workers, aides, chaplains, and other interdisciplinary team members.
• Conduct routine patient visits and adjust care plans as needs change.
• Provide education, guidance, and support to patients and their families.
• Perform clinical pronouncements and offer compassionate support during end-of-life transitions.
• Maintain accurate and timely clinical documentation.
• Supervise and support Home Health Aides in accordance with state, federal, and company guidelines.
