Formación

• Ability to apply SBAR communication for care planning.
• Understanding of safety and infection control in a hospital setting.

Responsabilidades

• Perform holistic patient assessments across physical, psychosocial, spiritual, and education needs.
• Coordinate and revise multidisciplinary care plans with patients and families.
• Communicate plans using SBAR and participate in handoffs.
• Lead discharge planning and relevant referrals.
• Provide ongoing patient/family education and document care in EMR.
• Respond to emergencies and maintain safe care environments.
