• Develops, communicates, and revises the plan of care with SBAR during shifts.
• Delivers education to patients and families and documents care plans in the EMR.
• Collaborates with physicians and team to implement evidence-based care.
• Prioritizes safety and infection control in all patient interactions.
• Assists in discharge planning and care transitions with appropriate referrals.
