Job Description

How you’ll make an impact in this role

• Deliver personalized patient care by executing treatment plans, carrying out physician orders, and tracking real-time condition changes to ensure clinical safety.
• Drive multidisciplinary collaboration as the primary care coordinator, keeping physicians, nurses, and allied health teams aligned on patient goals.
• Facilitate seamless discharge transitions by evaluating ongoing care needs with the care team to ensure patients return home safely and with complete clarity.

What minimum qualifications you’ll need

Licensure / Certification / Registration:

Required Credential(s):

• Registered Nurse obtained prior to hire date or job transfer date.
• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.

Education:

Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
