Job summary

Lavaca Bay Nursing and Rehabilitation Center seeks a skilled RN Charge Nurse to ensure high-quality care aligned with resident assessments and individual plans. You will monitor rounds, administer medications, and report changes, guiding caregivers and coordinating with physicians.

You will oversee aides, complete required nursing assessments, and communicate status updates to families while discharging residents per policy and supporting quality improvement initiatives.

Qualifications

• Active RN license in Texas in good standing.
• Experience in a skilled nursing facility preferred.

Responsibilities

• During rounds observes, assess, documents, and reports resident condition/changes.
• Assures implementation of care plans as directed.
• Administers medications as prescribed.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation with incoming and outgoing shifts.
• Oversees and monitors aides and initiates incident/accident reports.
• Discharge residents per facility policy and complete nursing assessments prior to physician calls.
• Communicate with residents' designated contacts regarding status updates and changes.

The RN Charge Nurse will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care.Essential FunctionsDuring rounds observes, assess, documents, and reports resident condition/changesAssures implementation of care plans as directedAdministers medications as prescribedConduct rounds with physician and record visits in clinical recordsPerform narcotic reconciliation with incoming and outgoing shiftsIdentify significant changes in the condition of residents and takes necessary action per facility policies and proceduresOversees and monitors aidesInitiate and complete Incident and Accident Reports per facility policyDischarge residents per facility policyMust complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident conditionCommunicate with resident’s point of contact when they ask for a status update or if there is a change in conditionOther special projects or functions can be assigned at times by the Director of Nurses and/or the AdministratorEducational/Training RequirementsHas completed a RN program at an accredited vocational school, college, or universityLicensing RequirementsMust be an active Registered Nurse in the state of Texas in good standingExperience RequirementsExperience working in a skilled nursing facility is preferred.
