Job summary

Fort Worth Transitional Care Center is seeking an RN Charge Nurse to support quality resident care. The role involves rounds, medication administration, and documenting changes in resident status to guide care planning.

The ideal candidate holds an active Texas RN license and RN program completion, with preferred experience in a skilled nursing facility. Candidates should be prepared to follow facility policies and coordinate with physicians and staff.

Qualifications

• Has completed an RN program at an accredited vocational school, college, or university.
• Must be an active Registered Nurse in the state of Texas in good standing.
• Experience working in a skilled nursing facility is preferred.

Responsibilities

• Observe, assess, document, and report resident condition changes.
• Administer medications as prescribed.
• Perform rounds with physician and record visits in clinical records.
• Identify significant changes and take action per facility policies and procedures.
• Oversees and monitors aides.
• Discharge residents per facility policy.
• Complete Nursing Assessment for Physician Calls prior to calling physician.
• Communicate with resident's point of contact for status updates or changes.
• Complete Incident and Accident Reports per facility policy.

The RN Charge Nurse will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care.Essential FunctionsDuring rounds observes, assess, documents, and reports resident condition/changesAssures implementation of care plans as directedAdministers medications as prescribedConduct rounds with physician and record visits in clinical recordsPerform narcotic reconciliation with incoming and outgoing shiftsIdentify significant changes in the condition of residents and takes necessary action per facility policies and proceduresOversees and monitors aidesInitiate and complete Incident and Accident Reports per facility policyDischarge residents per facility policyMust complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident conditionCommunicate with resident’s point of contact when they ask for a status update or if there is a change in conditionOther special projects or functions can be assigned at times by the Director of Nurses and/or the AdministratorEducational/Training RequirementsHas completed a RN program at an accredited vocational school, college, or universityLicensing RequirementsMust be an active Registered Nurse in the state of Texas in good standingExperience RequirementsExperience working in a skilled nursing facility is preferred.
