Job summary

Regency Integrated Health Services is seeking an RN Charge Nurse to help provide high-quality care promoting residents’ physical, mental, and psychological well-being. You will perform rounds, assess residents, document changes, and ensure care plans are followed, including medication administration and reporting to physicians as needed.

• Has completed an RN program and holds an active Texas RN license in good standing.
• Experience in a skilled nursing facility is preferred.
• Ability to perform nursing assessments and develop/modify care plans.

Responsibilities

• During rounds, observe, assess, document, and report resident condition changes.
• Ensure care plans are implemented as directed and administer medications as prescribed.
• Conduct rounds with physicians and document visits; perform narcotic reconciliation.
• Identify changes in resident condition and take action per policy.
• Oversee aides and communicate with residents' contacts about status updates.
• Complete Nursing Assessments before physician calls and follow facility procedures.

The RN Charge Nurse will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care.Essential FunctionsDuring rounds observes, assess, documents, and reports resident condition/changesAssures implementation of care plans as directedAdministers medications as prescribedConduct rounds with physician and record visits in clinical recordsPerform narcotic reconciliation with incoming and outgoing shiftsIdentify significant changes in the condition of residents and takes necessary action per facility policies and proceduresOversees and monitors aidesInitiate and complete Incident and Accident Reports per facility policyDischarge residents per facility policyMust complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident conditionCommunicate with resident’s point of contact when they ask for a status update or if there is a change in conditionOther special projects or functions can be assigned at times by the Director of Nurses and/or the AdministratorEducational/Training RequirementsHas completed a RN program at an accredited vocational school, college, or universityLicensing RequirementsMust be an active Registered Nurse in the state of Texas in good standingExperience RequirementsExperience working in a skilled nursing facility is preferred.
