Job summary

Windsor Nursing and Rehabilitation Center of San D is seeking an RN Charge Nurse to ensure quality resident care and timely assessment updates. You will observe, document, and respond to changes in resident conditions, administer medications, and coordinate with physicians and nursing staff.

Responsibilities include overseeing aides, maintaining care plans, completing nursing assessments, and timely reporting per facility policy. Texas licensure is required, with preferred SNF experience.

Qualifications

• RN trained and licensed to practice in Texas.
• Experience coordinating care plans and documenting resident changes.
• Ability to supervise aides and communicate with families.

Responsibilities

• During rounds observes, assesses, documents and reports resident condition/changes.
• Assures implementation of care plans as directed.
• Administers medications as prescribed.
• Conducts rounds with physician and records visits in clinical records.
• Performs narcotic reconciliation with incoming and outgoing shifts.
• Identifies significant changes and takes action per policies.
• Oversees and monitors aides.
• Initiates and completes Incident and Accident Reports per policy.
• Discharges residents per policy.
• Completes Nursing Assessments prior to physician calls.
• Communicates with resident’s point of contact about status changes.
• Participates in other special projects as assigned by Director of Nurses/Administrator.
