Job summary

Brenham Nursing And Rehabilitation Center in Brenham, TX seeks a RN Charge Nurse to ensure quality resident care and adherence to care plans. You will observe, assess, document, and report changes during rounds and administer medications per physician orders.

Active Texas RN license required; RN program completed. Experience in skilled nursing preferred. You will supervise aides, reconcile narcotics, report incidents, discharge residents, and coordinate with physicians and families.

Qualifications

• Has completed a RN program at an accredited vocational school, college, or university.
• Must be an active Registered Nurse in the state of Texas in good standing.
• Experience working in a skilled nursing facility is preferred.

Responsibilities

• During rounds observes, assesses, documents, and reports resident condition changes.
• Assures implementation of care plans as directed and administers medications as prescribed.
• Conducts rounds with physician and records visits in clinical records.
• Oversees narcotic reconciliation with incoming/outgoing shifts.
• Identifies significant changes and takes action per facility policies.
• Oversees and monitors aides and initiates incident/accident reports.
• Discharges residents per facility policy and completes Nursing Assessments prior to physician calls.
• Communicates with resident's point of contact about status changes.
