Job summary

Memorial City Nursing and Rehabiliation Center is seeking an RN Charge Nurse to lead rounds, assess residents, and ensure care plans are followed. You will administer medications, coordinate with physicians, and complete nursing assessments as conditions change.

The role requires Texas RN licensure and prior skilled nursing facility experience. Excellent communication with families and staff is essential for quality resident care and safety.

Qualifications

• RN program completed at an accredited institution.
• Active Texas RN license in good standing.
• Experience in skilled nursing facility preferred.
• Familiarity with resident assessments and care planning.

Responsibilities

• Observe, assess, document, and report resident condition changes during rounds.
• Administer medications as prescribed and ensure accuracy in transcriptions.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation with incoming/outgoing shifts.
• Identify significant changes in residents' condition and take action per policy.
• Oversee and monitor nursing aides and staff.
• Complete Nursing Assessments prior to physician calls and update status to families as needed.
• Discharge residents per facility policy and coordinate with care team.

The RN Charge Nurse will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care.Essential FunctionsDuring rounds observes, assess, documents, and reports resident condition/changesAssures implementation of care plans as directedAdministers medications as prescribedConduct rounds with physician and record visits in clinical recordsPerform narcotic reconciliation with incoming and outgoing shiftsIdentify significant changes in the condition of residents and takes necessary action per facility policies and proceduresOversees and monitors aidesInitiate and complete Incident and Accident Reports per facility policyDischarge residents per facility policyMust complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident conditionCommunicate with resident’s point of contact when they ask for a status update or if there is a change in conditionOther special projects or functions can be assigned at times by the Director of Nurses and/or the AdministratorEducational/Training RequirementsHas completed a RN program at an accredited vocational school, college, or universityLicensing RequirementsMust be an active Registered Nurse in the state of Texas in good standingExperience RequirementsExperience working in a skilled nursing facility is preferred.
