Job summary

Ascension Living in Wichita, KS, is seeking a Home Care IDT nurse to participate in developing, implementing, and evaluating the interdisciplinary team plan of care for participants in their homes and community settings.

You will attend IDT care plan meetings and family meetings as applicable, perform assessments in participants' residences or clinics, and educate participants, caregivers, and staff, including end-of-life discussions, with travel to homes or LTAC/LTC facilities as needed.

Responsibilities

• Participate in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care and update changes as necessary.
• Attend Interdisciplinary Team (IDT) care plan meetings and Participant/Family meetings as applicable. May perform assessments in a variety of settings including participant's place of residence, PACE Clinic and/or Adult Day Center.
• Develop and provide education to participants, caregivers, and associates as necessary including end of life discussions. May require travel to participant's place of residence, long term care and assisted living facilities.
• Assess participants in their homes for home care needed to support their independence living in the community.
