Job summary

HopeWest is seeking a Registered Nurse (Variable) for palliative care in Meeker. You will deliver comprehensive nursing care, coordinate chaplains, social workers, and physicians, and support patients and families in meaningful ways.

The role requires at least 2 years of clinical nursing experience, a Colorado RN license, CPR, and a valid driver’s license. PRN shifts include weekend commitments and flexible scheduling.

Qualifications

Education: Graduated from an accredited school of nursing; Bachelor’s degree preferred.
Experience: Minimum of two years clinical nursing; hospice/palliative care or related settings preferred.
Licenses: Colorado RN license, CPR, driver’s license, and auto insurance maintained throughout employment.

Responsibilities

Provide comprehensive palliative nursing care through assessment, care planning, and symptom management.
Serve as case manager for an assigned patient caseload and coordinate care across settings.
Collaborate with physicians, community partners, and interdisciplinary team to support quality of life for patients with serious illness.
Maintain EMR documentation and educate patients and families.
