You will provide professional nursing care in alignment with physician plans, assess patient needs, implement treatment, and document progress while collaborating with a multidisciplinary team.

Responsibilities include initial assessments, discharge planning, patient education, safety, and coordinating care with families.

Qualifications

• Professional nursing theory and practice knowledge.
• Current NYS Registered Nurse license.

Responsabilités

• Collect patient health data with patient/family input.
• Perform initial assessment based on presentation.
• Document nursing assessments and plans of care.
• Incorporate discharge planning into admission process.
• Collaborate with multidisciplinary team to meet patient needs.
• Provide education to patient and family.
• Maintain patient safety and report changes.
