Responsibilities

• Maintain ongoing communication with patients, nursing colleagues and the health care team about patient status and plans.
• Administer medications using bar-scanning and documentation technologies for safe delivery.
• Document each patient’s assessments, plan of care, interventions and evaluation in the Point-of-Care system.
• Provide patient and family education to optimize health and disease management.
• Coordinate multi-disciplinary discharge planning for safe transitions and continuum of care.
