Responsibilities

• Deliver nursing practices according to evidence-based research, policies and standards to provide safe, individualized patient care.
• Ongoing communication with patients, nursing colleagues and health care team regarding patient status and plans.
• Administer medications using bar-scanning and documentation technologies for safe delivery.
• Document patient assessments, plans, interventions and evaluations in point-of-care systems.
• Provide patient and family education to optimize health and disease management.
• Coordinate multi-disciplinary discharge planning for safe transition of care.
