Job summary

Ascension is seeking an experienced Registered Nurse for the Middleburg, FL area at Ascension St. Vincent's Clay County Hospital. The role is full-time, dayshift, with on-site work and collaboration with multidisciplinary teams to deliver patient-centered care.

You will follow clinical standards, monitor patient progress, coordinate care plans, and support safe discharge planning. Florida RN licensure and BLS are required; a diploma or equivalent licensure is needed to begin.

Qualifications

• BLS Provider within 30 days of hire. American Heart Association or American Red Cross accepted.
• RN credentialed from the Florida Board of Nursing obtained prior to hire.

• Drive the implementation of patient care plans, pro-actively monitoring status and documenting progress to ensure optimal outcomes.
• Act as the primary coordinator for multidisciplinary teams, facilitating seamless communication and well-integrated treatment.
• Execute physician and nursing orders with precision, serving as a critical link in the clinical decision-making process.
• Lead the discharge planning process in collaboration with the health-care team to ensure a safe and successful transition for every patient.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.

Education

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
