Job summary

Bearmountainhc, located in West Roxbury, MA, is seeking a dedicated RN MDS Coordinator for a 32-hour work week. This position primarily focuses on care planning and requires collaboration with an interdisciplinary team.

The ideal candidate will be a graduate of an accredited nursing program with current Massachusetts licensure and preferably has experience in long-term care, Medicare/Medicaid reimbursement, and care planning.

Qualifications

• Graduate of an accredited nursing school with current LPN/RN licensure in Massachusetts.
• One year of Long-term care experience is required.
• Experience with Medicare/Medicaid and clinical resources is preferable.

Responsibilities

• Follow the MDS calendar to schedule resident care conferences.
• Complete comprehensive care plans efficiently in PCC.
• Coordinate care plan meetings with the interdisciplinary team.

Job description

Care Village at Parkway, a skilled nursing facility in West Roxbury, MA, is currently looking for a 32-hour per week RN MDS Coordinator with a special interest in care planning to join their team.

Duties include but are not limited to:

• follow the MDS calendar to schedule resident care conferences,
• complete comprehensive care plans in PCC in a timely manner;
• utilize information from the medical record,
• resident and staff interviews as well as quarterly assessments, consultant reports, etc.
• coordinate care plan meetings with interdisciplinary team, resident and resident representative,
• update existing care plans as needed.

Requirements:

• Graduate of an accredited school of nursing with current LPN/RN licensure in Massachusetts
• One year of Long-term care experience
• Experience with Medicare/Medicaid reimbursement, MDS completion, clinical resources utilization, and/or case management is highly desirable
