You will complete MDS assessments, monitor timelines, coordinate care plans, and ensure compliance with regulatory guidelines while supporting a resident-centered approach.

Qualifications

• Must hold MA RN license.

Responsibilities

• Completes MDS assessments and care plans for all residents assigned.
• Monitors MDS timelines and regulatory deadlines.
• Advises supervisor of incomplete/untimely assessments by other disciplines.
• Ensures timely completion of MDS/RAPs/Triggers sheet for residents.
• Initiates care plans and coordinating activities to achieve best outcomes.
• Generates and distributes monthly care plan calendars.”
• Maintains and updates all care plans quarterly and adjusts as needed.
• Reviews incidents and 24-hour reports to reflect changing needs.
