RegalCare at Greenfield in Wakefield, MA is seeking an experienced MDS Coordinator. A licensed RN or LPN will ensure accurate MDS assessments, coding, and regulatory compliance, coordinating with the interdisciplinary team to plan care and optimize reimbursement.

Qualifications

• Current RN or LPN license in Massachusetts (required).
• Minimum 1 year of MDS experience in a skilled nursing facility preferred.
• Strong knowledge of RAI process, PDPM, and Medicare/Medicaid guidelines.
• Excellent attention to detail, communication, and organizational skills.
• Ability to work independently and as part of an interdisciplinary team.

A well-established skilled nursing and rehabilitation center in the Wakefield MA area is seeking an experienced and detail-oriented MDS Coordinator to join our team full-time. The ideal candidate will be a licensed nurse with strong clinical assessment and documentation skills who can ensure accurate and timely completion of MDS assessments and compliance with all state and federal regulations.
