Pathways Healthcare, a leading home health and hospice provider, is seeking a Registered Nurse or Licensed Practical Nurse for Home Health in the Malden, Melrose, Saugus area. Per-visit compensation is offered, with responsibilities spanning initial assessments, medication administration, and collaborative care planning with physicians and interdisciplinary teams.

Formación

• Must hold an active RN or LPN license in the state where the position is based.
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsabilidades

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.

Job Description:

• Role: Registered Nurse/Licensed Practical Nurse - Home Health
• Rate: Per Visit
• Service Area: Malden, Melrose, Saugus areas.

Qualifications:

• Must hold an active RN or LPN license in the state where the position is based. LPNs are encouraged to apply.
