Job summary

Pathways Healthcare is seeking a Registered Nurse or Licensed Practical Nurse for Home Health in Massachusetts. You will deliver patient-centered care across Beverly, Danvers, Lynn, Peabody and surrounding areas, following physician orders and collaborating with an interdisciplinary team.

The role emphasizes clinical judgment, documentation, and compassionate patient interactions in home settings. A valid RN/LPN license, BLS, and a driver’s license with reliable vehicle are required.

Qualifications

• Active RN or LPN license in the state where based.
• Two+ years nursing experience, preferably in geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Working knowledge of healthcare laws and regulations.
• Current BLS CPR certification.
• Valid driver’s license and own insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial patient and family assessments to determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, NPs, and interdisciplinary teams to create robust Plans of Care.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them as part of care plan.

Job Description:

• Role: Registered Nurse/Licensed Practical Nurse - Home Health
• Rate: Per Visit
• Service Area: Beverly, Danvers, Lynn, Peabody, Middleton, Salem, Swampscott areas.

Qualifications:

• Must hold an active RN or LPN license in the state where the position is based. LPNs are encouraged to apply.
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
