Job summary

Pathways Healthcare in Massachusetts seeks an RN/LPN for Home Health in the Arlington area and surrounding communities. The role includes patient assessments, developing care plans, medication administration, and collaboration with physicians and teams.

We value bedside manners, strong communication, and adherence to healthcare regulations with a current BLS CPR certification and valid driver’s license. Experience in geriatrics is preferred; LPNs are encouraged to apply.

Qualifications

• Active RN or LPN license in the state where based.
• Preferably 2+ years nursing experience in geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Current BLS CPR certification and valid driver's license with own vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed.
• Assess patients and families to determine home care needs.
• Perform thorough physical assessments and history gathering.
• Develop and revise plans of care with evolving patient needs.
• Collaborate with physicians and interdisciplinary teams on plans of care.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them.

Job Description:

• Role: Registered Nurse/Licensed Practical Nurse - Home Health
• Service Area: Arlington, Belmont, Lexington, Waltham, Watertown areas.

Qualifications:

• Must hold an active RN or LPN license in the state where the position is based. LPNs are encouraged to apply.
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities:

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
