Job summary

Pathways Healthcare, Llc is seeking a Registered Nurse or Licensed Practical Nurse for full-time home health care in the Boston area. Office is in Westwood, MA with service coverage in Dorchester and surrounding areas. Schedule is Monday–Friday, 8:30 AM to 5:00 PM.

Ideal candidates have 2+ years of nursing experience, a nursing degree, and proficiency with EMR systems. Training programs available for new nurses; drivers licensed and insured vehicles preferred.

Qualifications

• 2+ years' nursing experience, preferably in geriatrics.
• New nurses welcome with training program.

Responsibilities

• Assess and evaluate patient status and plan of care.
• Develop care plans with patient and family involvement.
• Administer medications and treatments as prescribed.
• Maintain accurate clinical notes and report changes to physician.
• Provide safe patient environment and participate in team meetings.

Job Description:

• Role: Registered Nurse/Licensed Practical Nurse
• Salary: Commensurate with experience
• Main Office location: Westwood, MA
• Service Area: Dorchester and Surrounding Areas
• Schedule: Monday - Friday (8:30-5:00pm)
• Job Type: Full Time

Qualifications:

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• New nurses, feel free to apply! We have a great training program.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities:

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.
