Pathways Healthcare is a leading home health and hospice provider, offering AI-assisted documentation to save clinicians 5–10 hours per week. The Licensed Practical Nurse - Home Health role serves Mansfield, Plainville, Norton, Foxboro and surrounding areas, with flexible scheduling and main office in Westwood, MA.

The ideal candidate holds an active nursing license in the state, with 2+ years in geriatrics, a nursing degree, strong bedside manner and communication skills, and a valid driver's

Qualifications

• Must hold an active Nursing license in the State where the position is based.
• Preferably 2+ years of nursing experience with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
