Pathways Healthcare in Westwood, MA is seeking a Registered Nurse - Home Health to join our physician- and nurse practitioner-led team. You will provide patient-centered care in the home, serving Newton, Allston, Brookline, Brighton and surrounding areas, Monday to Friday 8:30 AM to 5:00 PM.

We value clinical judgment and offer advanced AI-assisted documentation that saves time, with a focus on dignity and quality.

Formación

• 2+ years nursing experience, preferably in geriatrics.
• New nurses, feel free to apply! We have a great training program.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsabilidades

• Assesses and evaluates patient’s status by: writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.
• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.

Job Description:

• Role: Registered Nurse - Home Health
• Main Office location: Westwood, MA
• Service Area: Newton, Allston, Brookline, Brighton and Surrounding Areas
• Schedule: Monday to Friday (8:30-5:00pm)
• Job Type: Full Time

Qualifications:
