Job summary

Pathways Healthcare, Llc is seeking a Licensed Practical Nurse - Home Health to join our patient‑centered care team. This role offers a per‑visit compensation basis and will primarily serve the Westwood, MA area with a broad service region including Mansfield, Plainville, Norton, Foxboro and surrounding communities.

The schedule is flexible to meet patient needs, with support from a physician‑led and nurse practitioner‑led leadership.

Qualifications

• Active Nursing license in the state where based.
• 2+ years nursing experience, geriatrics preferred.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Knowledge of healthcare laws and regulations.
• Current BLS CPR certification.
• Valid driver’s license and own reliable, insured vehicle.

Responsibilities

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
