Signature HealthCARE of Roanoke Rapids is actively seeking experienced RNs to join a 108-bed facility providing short-term rehabilitation and long-term care. You will deliver direct nursing care, supervise aides, and coordinate resident plans within a team focused on resident-centered care and quality outcomes.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Familiarity with EHR systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) with dignity and safety.
• Supervise and direct nursing assistants and medication aides during shifts.
• Monitor residents’ status and communicate changes to physicians and families.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy and procedure.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and regulatory surveys.
