Signature HealthCARE of Roanoke Rapids is seeking a dedicated RN for direct patient care and leadership within a 108-bed facility in Roanoke Rapids. You will administer medications, assist with ADLs, and supervise nursing staff to maintain high-quality resident care.

Qualifications

• Active RN license in the state of practice.
• Minimum one year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during the shift.
• Monitor residents’ status and notify physicians of changes.
• Develop and review individualized resident care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and training.
• Perform other duties as assigned.
