Signature HealthCARE of Roanoke Rapids seeks an LPN/LVN to provide direct nursing care, administer medications, and supervise nursing assistants on shift. Join a 108-bed facility delivering rehab and long-term care in a supportive, team-driven environment.

You will collaborate with an interdisciplinary team, develop resident care plans, document accurately, and ensure safety and dignity in daily activities.

Formación

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsabilidades

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs to ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants during shifts per policies.
• Monitor residents’ status and notify physician of changes.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities, staff education, and surveys.
• Perform other duties as assigned by the Director of Nursing.
