Job summary

Signature HealthCARE of Roanoke Rapids is seeking an experienced Registered Nurse to join our 108-bed facility. You will provide direct nursing care, oversee nursing assistants, and coordinate admissions, transfers, and discharges while ensuring comfort, dignity, and safety for residents.

The role emphasizes teamwork with the interdisciplinary staff, accurate clinical documentation, adherence to federal and state regulations, and ongoing participation in quality assurance activities.

Qualifications

• Current, active RN license in the state of practice (North Carolina).
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies.
• Monitor and observe residents’ physical and emotional status; notify the attending physician of changes.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges.
• Participate in quality assurance activities, staff education, and regulatory surveys.
