Job summary

West Baltimore PACE Community Coalition LLC is seeking a Registered Nurse to provide skilled nursing care to participants under the Clinic Manager’s supervision in Baltimore, MD. You will work closely with the Interdisciplinary Team and family members to achieve the care plan goals and educate patients and caregivers.

The ideal candidate holds Maryland RN licensure, with at least 1 year of healthcare experience with frail or elderly populations.

Qualifications

• Graduate of an accredited Registered Nurse Program and licensed in Maryland.
• One year of health care experience with frail/elderly populations preferred.
• Efficient with computer applications including email, internet, timekeeping, and EMR.

Responsibilities

• Provide skilled nursing care under supervision of the Clinic Manager.
• Collaborate with Interdisciplinary Team and families/caregivers to meet care plan goals.
• Perform blood draws, IV therapies, wound care, and medication reconciliation.
• Document results in EMR and communicate orders to providers/pharmacy.
• Maintain confidentiality and comply with safety policies and OSHA guidelines.
• Continuously participate in trainings and quality assurance activities.
