Job summary

PacificSource PACE LLC seeks a dedicated registered nurse to provide direct clinic nursing care for older adults in a geriatric PACE setting. You will assess, treat, educate, and coordinate care with an interdisciplinary team to ensure safe, participant-centered services.

The role includes vaccine administration, documentation, and cross-coverage for home nursing as needed, with travel about 10% of the time. RN license and CPR required.

Qualifications

• Registered Nurse with current, unrestricted state license and an associate’s degree is required.
• CPR certification required at time of hire or within three months.
• Strong interpersonal and communication skills; ability to work in interdisciplinary team.

Responsibilities

• Develop, implement, and coordinate individualized nursing care for PACE participants in the clinic.
• Conduct clinic-based nursing assessments and support follow-up plans.
• Provide direct nursing care: vital signs, meds, wound care, injections, immunizations.
• Collaborate with physicians, NPs, and staff to ensure coordinated patient care.
• Educate participants and caregivers on treatments, meds, self-care and prevention.
• Support vaccine programs: planning, outreach, administration, and documentation.
• Monitor supplies and vaccines; coordinate inventory and readiness.
• Act as clinical resource and assist with care planning and processes.
• Participate in interdisciplinary team and quality initiatives.
• Provide on-call/urgent coverage as needed.
