Job summary

Concordia Visiting Nurses is seeking a dedicated Registered Nurse to join our home health team. You will deliver compassionate care in patients' homes, upholding our Christian values and mission.

You will assess, plan, and coordinate care with physicians, patients, and families, empowering patients to manage their health and recover at home. Independence and strong clinical skills are essential.

Qualifications

• Minimum one year of recent clinical RN experience recommended.
• Home care experience recommended.
• Adaptability, compassion, and patient-centered care.

Responsibilities

• Conduct thorough assessments and develop individualized care plans with physicians, patients, and families.
• Deliver treatments, IV starts, chest tube management, and wound care.
• Educate and empower patients and families to manage health conditions and build confidence in self-care.
• Utilize strong clinical assessment skills to monitor patient conditions and identify changes.

What you'll do as a Registered Nurse RN:

As a Home Health RN, you'll be at the heart of this mission.
